Acquired Penile Curvature / Peyronie’s Disease

Contents

Peyronie’s Disease is acquired penile curvature; which is when you have previously had a straight penis in the erect state, but a curvature started developing at some point in time, ending up in a curved penis / bent penis. This is mostly due to Peyronie’s plaques.

Peyronie’s disease is the deposition of inelastic tissues, usually to rebuild and bridge a gap at the site of injury, whether you know of or you don’t, since it may be very subtle but repetitive, due to slight bending of the erect penis upon pressure, during intercourse.  Fibrous tissue in the penis leads to shortening, curvature and loss of erection in some cases, being inelastic. Fibrosis may also occur due to repeated use of intra corporal injection for the treatment of erectile dysfunction. Diabetes is a common predisposing factor.

In Peyronie’s disease, inelastic fibrous tissue is deposited on the walls (tunica albuginea) of the erection cylinders (corpora cavernosa). The fibrous tissue deposits are called “Peyronie’s Plaques”.

So I have Peyronie’s Disease. What do I do?

Peyronie’s disease assumes a progressive course over one to two years, with some pain in the penis and increasing curvature and shortening. After one to two years, it stabilizes and progresses no more, and pain fades away. Before stabilization, medical treatment is advised to limit progression. After stabilization, surgery can be resorted-to, to correct the resultant deformity, if necessary.

Medical treatment for Peyronie’s disease is either oral tablets or intra-plaque injections, which are shots given in the Peyronie’s plaque by an expert in the field. The main aim of oral pills for Peyronie’s curvature is to slow down the progress of Peyronie’s curvature in the first one or two years, until stabilization. Some medical reports indicate some degree of success for Peyronie’s pills in decreasing the deformity and the curve in the penis. Injections into Peyronie’s plaques have met higher success, though with still modest results except in milder cases in the first phase of the disease.

The most recent treatment for Peyronie’s curvature is collagenase injection. This is an enzyme that breaks down fibrous tissue strands. It is injected into the Peyronie’s plaque in the hands of an expert, followed by modelling of the penis. This is repeated over several cycles. Side effects of collagenase are few, including allergic reactions and some reports of penile fracture due to weakening of the tissues at the site of injection.

Surgery is the main treatment method for moderate to severe degrees of Peyronie’s disease. Surgical treatment depends on the degree of deformity, and whether or not it is associated with erectile dysfunction.

Peyronie’s Surgery

If Peyronie’s disease is associated with severe erectile dysfunction, or if penile curvature is too severe, penile prosthesis implantation is warranted along with correction of curvature. Click the picture for full information on how to restore full erection with a penile implant :

However, if there is no ED, erection is rigid, and the problem is only penile curvature, surgical correction is performed by one of two methods, depending on the degree of curvature:

Mild to moderate penile curvature due to Peyronie’s disease, with adequate length: Correction of penile curvature by Shortening Techniques. Click the picture for full information:

Any of those technique can be coupled with surgical elongation of the penis
